DOH: Ongoing ePOC Issue

The electronic Plan of Correction (ePOC) system is the primary vehicle by which the NYS Department of Health communicates survey information to nursing homes. Survey reports and plans of correction are communicated and warehoused in the ePOC system.

Following a recent system upgrade in late April 2025, the Department identified an error that is incorrectly translating some survey data from Aspen to ePOC. The Department is working with the federal Aspen technical experts to troubleshoot and correct the issue. However, there is no estimated timeframe for remediation.

In the interim, the Department’s survey teams will communicate survey information via the Secure File Transfer application on the Health Commerce System. Plans of correction may be communicated back to the Department via the Secure File Transfer application.

Individuals in the Nursing Home Administrator or Administrator Backup roles on the Health Commerce System can receive the Statements of Deficiency via the Secure File Transfer Process. These individuals will be proactively outreached when the file is sent.

The Administrator or Administrator Backup must contact the regional office to advise when they submit their plan of correction via Secure File Transfer.

If the facility’s response is not received within ten days from the date the SOD was sent, the regional office will outreach the Nursing Home Administrator or Administrator Backup.

Be reminded that Secure File Transfer packages expire after 14 days.

Once the ePOC system returns to proper function, the Department will notify providers of the return to normal work process and procedure. The Department will then translate the information into ePOC using the original survey letter and dates.
