ACF 4th Quarter Statistical Report Due January 31st
The Department of Health has issued the attached DAL DACF 24-10 2024 Adult Care Facility 4th Quarter Statistical Information Report. The report, which is accessible in HCS, is due by January 31, 2025.
All Adult Care Facilities licensed by the New York State Department of Health are required to complete the 2024 4th Quarter Statistical Information Report, reflecting the period from October 1, 2024 through December 31, 2024. This Quarterly Statistical Information Report must be submitted no later than January 31, 2025. Faxed or printed copies will not be accepted. Please note that failure to submit this Quarterly Statistical Information Report by January 31, 2025, may result in enforcement action, including the imposition of civil penalties.
