HERDS Critical Asset Survey – Shutdown Coming on 12/20/24

This message is to inform all Health Care Facilities (HCFs) – hospitals, nursing homes (NHs) and adult care facilities (ACFs) – that the HERDS Critical Asset Survey will be temporarily shut down – starting at 5:00pm Friday, December 20, 2024, and continuing through January 1, 2025.

The Critical Asset Survey will be reactivated on January 1, 2025 – in conjunction with the 2025 compliance period for the Facility Evacuation Planning Application (FEPA) in New York City. A Dear Administrator Letter (DAL) regarding the Facility Evacuation Planning Application will be forthcoming to HCFs in NYC.

All Health Care Facilities are required to review their previously submitted data, update any information that has changed and resubmit the Critical Asset Survey data.
