2017-19 ALP Minimum Wage Survey Deadline

A reminder that the 2017-19 ALP Minimum Wage Survey and Attestation are due TODAY, Wednesday, January 6, 2021.

Good Afternoon,

The 2017-2019 ALP Minimum wage survey and attestation (both attached) are due no later than January 6, 2021 (Wednesday)

If you choose NOT to submit this survey, there will be no additional funds provided from the State to support minimum wage costs during the referenced time periods.  If your facility does not have any employees below the minimum wage, or if you are choosing not to submit, we still ask that you complete the OPT OUT section and attestation. We appreciate your cooperation.

If you have any questions regarding the survey, please reply to the email above and include your provider ID, agency name and contact information.
