Reminder – Quarterly Statistical Report DUE October 31st
The Department of Health issued the attached DAL DACF 24-07 2024 Adult Care Facility 3rd Quarter Statistical Information Report on October 1, 2024. The report, that is accessible in HCS, is due by October 31, 2024.
All Adult Care Facilities licensed by the New York State Department of Health are required to complete the 2024 3rd Quarter Statistical Information Report, reflecting the period from July 1, 2024, through September 30th, 2024. This Quarterly Statistical Information Report must be submitted no later than October 31, 2024. Faxed or printed copies will not be accepted. Please note that failure to submit this Quarterly Statistical Information Report by October 31, 2024 may result in enforcement action including the imposition of civil penalties.
