ALP 2020-2021 Minimum Wage Survey + Attestation due March 2nd, 2022

NYSHFA|NYSCAL would like to remind you that the2020/2021 ALP Minimum Wage survey and attestation is due no later than March 2, 2022. Attached is the 2020-2021 ALP Minimum Wage survey and attestation. During a phone call with the Department of Health, NYSHFA|NYSCAL and the other associations advocated to allow providers 30 days to respond which DOH agreed to.

The attached instructions will provide you with guidance on filling out the 2020-2021 Minimum Wage spreadsheet.

Please note, that if you choose NOT to submit this survey, there will be no additional funds provided from the State to support minimum wage costs during the referenced time periods. If your facility does not have any employees below the minimum wage, or if you are choosing not to submit, DOH is requesting that you complete the OPT OUT section and attestation.
