DOH: Mandatory Vaccine Tracker Reporting

You are receiving this note as an enrollee in the NYS COVID-19 Vaccination Program and as a registrant in the NYS Immunization Information System (NYSIIS) or the City Immunization Registry (CIR).  This is a reminder that all providers receiving COVID-19 vaccine in New York State must complete the NYS Vaccine Tracker every Monday and Thursday by 10 am.

Completion of the NYS Vaccine Tracker is a mandatory program requirement, and any provider failing to report timely and accurately will no longer be allocated vaccine.

This reporting requirement is applicable to ALL providers in receipt of COVID-19 vaccines irrespective of the source or allocating entity, including: direct allocations from the State of New York or New York City, direct allocations from the Federal Government, and any redistributions from other providers.

The information you report to the Vaccine Tracker is used by the State to analyze vaccination throughput and facility specific vaccine inventory, and to determine eligibility for future vaccine allocations.

The reporting requirement is not applicable to providers that have not received vaccine and/or previously reported depletion of prior allocations.
