HERDS Survey Webinar and Changes
As of October 7th, the HERDS survey will be revised to eliminate the PPE section questions. Questions related to the PPE will be added once a month starting in November. Other changes will be announced on tomorrow’s webinar regarding direct care staffing, resources and booster shots.

Updates to the Nursing Home and Adult Care Facility Daily HERDS Survey
October 4, 2021
On Tuesday, October 5, 2021 the Department of Health will host a webinar from 10 a.m. to 11 a.m. for nursing home and adult care facility providers (ACF) to review updates to the Nursing Home and ACF Daily HERDS surveys. These changes will take effect on Tuesday, October 5, 2021 for nursing homes and Wednesday October 6, 2021 for adult care facilities.

Providers are reminded that they have a continued and expected obligation to ensure that all information submitted on the daily HERDS survey is accurate. Any questions related to this notification should be forwarded to covidnursinghomeinfo@health.ny.gov or covidadultcareinfo@health.ny.gov.
