NYSDOH Information on MEMORANDUM OF UNDERSTANDING (MOU) REGARDING COVID-19 VACCINE ADMINISTRATION

NYSDOH provided updated information on the MOU recently distributed to a select group of providers who dually enrolled in both the Federal Pharmacy Partnership for LTC Vaccination Program and the New York State COVID-19 Vaccination Program. The MOU was sent to those providers to ensure safe distribution and administration of the COVID-19 vaccine.

If the provider who received this MOU feels they cannot adhere to the criteria set forth in the MOU, the provider should note on the MOU: “Unable to Commit” and return the MOU UNSIGNED to the email address listed on the bottom of the MOU: Health.sm.Covid19VaccineMOU@health.ny.gov
