Skilled Nursing Facility Reporting Guide

The NYS Department of Health requires nursing home facilities to have policies in place and staff trained on how to manage reportable incidents. This list of situations and/or incidents is intended to provide clarification and guidance on what incidents are considered reportable, the required timeframe to report and how incidents should be reported to appropriate agencies to assist providers to remain in regulatory compliance.
