Reminder if you have not yet completed your facility MDS certification – certifications for the July 2021 MDS collection are due NO LATER than COB this Friday, July 30th. Please remember to send your certifications electronically to MDSBRHCR@health.ny.gov. If you have already completed and sent your facility certification please disregard.
