eFINDS Survey for ACF and NH

The New York State Department of Health activated a Health Emergency Response Data System (HERDS) survey on Monday, August 15, 2022 for hospitals, nursing homes and adult care facilities (ACFs) in New York State on the evacuation of facilities during a disaster and the use of eFINDS. Facilities are asked to carefully review and answer all questions based on the global use of the system within the facility and not on any one user.  DOH indicated that the collection of this information will be used for application improvements and enhancements for training and to provide guidance.  Providers must have the survey completed, saved and submitted by November 1, 2022.
