Reminder- LHCSA/ALP LHCSA Statistical Report Due January 31st
As a reminder, the LHCSA/ALP LHCSA Statistical Report is due by January 31st. Please refer to the attached DAL with instructions for guidance.
Please note that if you did not submit the Short Form or full Statistical Report by December 31st, you will not be able to access the report and must contact the Department of Health.
