REMINDER: ACF 2nd QSIR due July 31st!
The Department of Health has issued the attached DAL DACF 25-05 2025 Adult Care Facility 2nd Quarter Statistical Information Report. The report, that is accessible in HCS, is due by July 31, 2025.
All Adult Care Facilities licensed by the New York State Department of Health are required to complete the 2025 2nd Quarter Statistical Information Report, reflecting the period from April 1, 2025 through June 30, 2025. This Quarterly Statistical Information Report must be submitted no later than July 31, 2025. Faxed or printed copies will not be accepted. Please note that failure to submit this Quarterly Statistical Information Report by July 31, 2025, may result in enforcement action, including the imposition of civil penalties.
