Reminder – Due June 28th – Healthcare Personnel Flu Vaccination Report

This is a reminder that the Healthcare Personnel Influenza Report is due June 28th. This is applicable to facilities covered under Title 10, Section 2.59 (10 NYCRR Section 2.59), which includes hospitals, nursing homes, home care, hospice, diagnostic and treatment centers, and adult day health care (ADHC) providers. This includes home care staff working in ALPs. This is NOT applicable to ACFs otherwise.

Notes:

• This report is found on HCS
• The flu vaccination status of personnel who worked for at least one day between Oct. 1, 2023 and March 31, 2024 are to be recorded.
