Electronic Incident Reporting System

REMINDER: effective June 1, 2021, facilities will be required to use the new HCS-based Incident Reporting Form to convey reportable incidents to the Department of Health.  The new system replaces the previous paper-based method for reporting incidents.   For your convenience, we are attaching DAL 21-11 Electronic Incident Reporting.
