Quarterly Statistical Information Report Due October 31

The NYSDOH has sent NYSCAL a request to remind Assisted Living Providers that the QSIR is due 10/31/2023. Just a reminder, the Quarterly Statistical Information Report (QSIR) is a HERDS-based survey used to collect a facility’s demographics for the specified quarter.  All Adult Care Facilities must complete the QSIR. The QSIR is required per 18 NYCRR §487.10(e)(2), §488.10(e)(4), and §490.10(e)(4).
