DOH: Pause in HERDS reporting – July 4th Holiday

The Department remains on alert regarding COVID-19 and appreciates the information provided through the various COVID-19 surveys. The Department will not require HERDS reporting for hospitals, nursing homes, adult care facilities, CHHAs, LHCSAs, and hospices on the following date: Tuesday, July 4. HERDS reporting will resume on Wednesday, 7/5/23. Please note: CHHAs, LHSCAs, and hospices must complete the COVID-19 survey on 7/5/2023. The additional survey questions scheduled to be collected in the 7/4/23 COVID-19 survey for the hospitals (PPE Inventory, workforce reduction and EO#4), nursing homes (PPE Inventory, workforce reduction and EO#4), and adult care facilities ( workforce reduction and EO#4) will be collected in the 7/5/2023 COVID-19 surveys. Please ensure that data submitted on Wednesday is complete.
