This is the link to the webinar presented by the Office of Aging & Long-Term Care – Division of Nursing Homes and ICF/IID Surveillance demonstrating new facility incident reporting requirements effective 10/24/22.

The Facility Incident Report Nursing Home Facility Incident Report | Survey Builder (ny.gov) must be used for all reportable incidents occurring after 12:00AM October 24, 2022.
The Investigative Report Nursing Home Investigative Report | Survey Builder (ny.gov) must be submitted within 5 days of any reportable incident occurring after 12:00AM October 24, 2022.
