NH/ACF Weekly Staff Testing Survey (Week 72)
DOH has finalized the Week 72 Staff Testing Survey. There are substantive changes to this week’s survey for skilled nursing facilities only. The survey for ACFs remains unchanged.
The Week 72 survey will be released (via IHANS alert through HCS) Wednesday, 9/29 at 9am. As always, the survey will close promptly Thursday, 9/30 at 5pm.
All facilities will still receive the normal IHAN alerts:
- Wednesday, September 29, 2021: 9am message with link to the survey; and
- Thursday, September 30, 2021: 12pm reminder that survey will close at 5pm.
Please note the following:
If you do not receive a confirmation email, then the system did not log your survey submission.

- Most respondents who do not receive confirmation emails have misspelled their email address in the survey.
- If resubmitting, refresh the webpage and re-enter the survey information.
- If problems persist, email DOH ASAP.
