As a reminder, the ACF Financial Reports are due by May 31st.
Attached you will find DAL 24-12 2023 ACF Annual Financial Report along with the following attachments:
- Enclosure I: Financial Report Companion Guide
- Enclosure II: CPA Statement – Report on the Audit of the Financial Statements
- Enclosure III: CPA Statement – Consolidated Financial Statements
Summary
The Department of Health has activated the 2023 Adult Care Facility Annual Financial Report on the Health Commerce System (HCS). All ACFs operating for at least six months in 2023, including those with operator changes among related facilities, must submit the report by May 31, 2025, or earlier for non-calendar fiscal year facilities as outlined in regulations.
Reports are filed via HCS by designated Financial Data Reporters and Submitters.
On the HCS Home Page, Click My Applications → click HERDS → click Data Entry and
then select an Activity → 2023 Annual Financial Report
All facilities must submit a Certification of Operation statement. In addition, facilities with 26 or more beds must also complete a CPA Certification. Failure to file by the deadline may result in penalties under New York State Social Services Law. ACFs should consult the Annual Financial Report Companion Guide before entering data.
