DAL 21-18 Enhancing the Quality of Adult Living (EQUAL) 21-22
The New York State Department of Health has issued DAL 21-18, Enhancing the Quality of Adult Living (EQUAL) for SFY 21-22, along with a recorded training link that discusses the application process and acceptable vs. non acceptable expenses. The DAL is attached, along with the exhibits and the training link. The application is electronic and was posted on the Health Commerce System (HCS) effective October 26, 2021.
The EQUAL Program application can be accessed by logging onto HCS at: https://commerce.health.state.ny.us. Please carefully review the instructions. Applicants electing to participate must complete Sections A-E, including all certifications in Section E, of the HCS application and must be submitted electronically by 5:00 pm on November 19, 2021. After this date and time, the application will no longer be available, and hard copies will not be accepted. Applications that do not include all required, complete materials by 5:00 pm on November 19, 2021 will be deemed incomplete and ineligible for funding.
