Reminder! 3rd Quarter Statistical Report

If you have not already submitted your 3rd Quarter Statistical Report, this is a reminder that they are due to the Department of Health by October 31, 2021.  The Department has indicated to NYSCAL that Facilities that fail to submit the survey timely are subject to citation and applicable enforcement.

I have attached the DAL along with instructions on how to complete the report and definition of terms.
