NYSDOH Collection and Submission Process

Each month NYSDOH will notify approximately 100 nursing home facilities in writing to complete the infection control inspection annual audit checklist for that month. Those 100 nursing homes receiving the letter will be notified by NYSDOH of the specific date when to return the completed checklist. NYSDOH will work on reviewing and responding to the previous month’s submissions. The January 2024 notification letters were sent out to the first 100 nursing homes on January 2, 2024. If you already submitted your facility’s infection control inspection annual audit checklist ahead of receiving the NYSDOH notification letter, NYSDOH will keep them for future review. This submission and review process will continue until all infection control inspection annual audit checklists have been received by NYSDOH.
