DOH: Correction – Healthcare Personnel Influenza Vaccination Report

This is an informational message from the New York State Department of Health regarding the mandated annual Healthcare Personnel Influenza Vaccination report. The previous message had errors in the attached document regarding the open and closing dates of the survey. This has been corrected in the document attached to this message. The report opened today, Friday, April 26th and will close on Friday, June 28th. We apologize for any confusion the previous message may have caused. Please refer to the attached document for further information and instructions for this year’s report.
