QISR 4th Quarter Report Due January 31

The Department of Health has issued the attached DAL 24-01 – 2023 Adult Care Facility 4th Quarter Statistical Information Report on January 3, 2023. The report, that is accessible in HCS, is due by January 31, 2024. 

All ACFs licensed by the New York State Department of Health are required to complete the 2023 4th Quarter Statistical Information Report (QSIR), reflecting the period from October 1, 2023, through December 31, 2023. Please note that failure to submit this QSIR by January 31, 2024 may result in enforcement action including the imposition of civil penalties.
